We, at the Jamestown Family Health Clinic are adapting to the constantly evolving situation to best meet your safety and healthcare needs. We are pleased to announce that we are expanding our ability to perform remote visits for our patients instead of physically coming into the clinic to be seen for an office visit.
We are expanding access so people have the option to have visits with a provider virtually/online to address their health issues, both acute and chronic, to minimize the disruption that Covid-19 has caused you and the healthcare system.
What is Tele-medicine?
Tele-medicine is an online virtual visit using your home computer or smartphone device to connect you to your healthcare without leaving your home.
You will be able to see, hear, and talk to a JFHC Provider through videoconferencing, from the comfort of your home.
Just like with any face-to-face appointment, you can have a family member, care provider, or friend accompany you if you wish.
For those unable to successfully connect , we can offer telephone only visits.
How to Schedule an Appointment
If you need an appointment, call the clinic the same way you always have and our team will assist you with scheduling these new telehealth visits.
We are committed to offering uninterrupted care for all our patients and these services are an exciting new way to help keep our patients, their families, as well as our staff safe.
Visit types include:
- Chronic disease management such as Hypertension, Diabetes
- Acute visits (Non-Chronic)
- Preventive services such as Medicare’s annual Wellness visits
- MAT/Chronic Pain Managment
- Behavioral Health
- Prescription refills
- Diabetes education
- Regular check-ins and follow ups.

If you need to use a smart phone, please download Zoom™ from your appropriate App Store
Either option will require a web-camera and microphone
We highly recommend you also have access to your MyChart account. This gives you easy and immediate access to after visit summaries, normal lab and imaging results; as well as request medication refills and ask your provider questions.
