We recognize that all patients have basic rights, and we are committed to honoring these rights. Likewise, The Jamestown Family Health Clinic has a right to expect reasonable and responsible behavior from patients, their relatives and friends.

Jamestown Family Health Clinic has a process in which patients are able to voice concerns without fear of jeopardizing their care. If you or your family member have concerns about medical care received, or received exceptional care you wish to recognize, please feel free to speak with any manager or complete the form below.  A member of our administrative staff will review and contact you. If you prefer to submit your feedback anonymously, feel free, however understand we will not able to respond.
