We are a patient centered program designed to help individuals who are diagnosed with diabetes to achieve and maintain good health and minimize complications. Our team of certified diabetic educators (CDE) along with primary care providers, and nursing staff work with individuals to understand the process of diabetes and how to prevent complications. They will work together with patients to identify the best medication options and help set individualized realistic goals.
